Please indicate the area(s) of your greatest discomfort by circling the affected region(s) on the body diagram provided and describe your symptoms as clearly as possible, including when they began, what aggravates or relieves them, and whether the pain radiates to other areas.
Your first appointment includes a comprehensive osteopathic evaluation to identify the source of your symptoms and determine whether Deep Tissue Release Therapy is appropriate.
The visit typically includes:
